A tiny hole in the end of a pen cap that appears to serve no purpose whatsoever.

What is actually happening

Pen caps are among the most commonly inhaled small objects, especially among children and habitual pen-chewers. A lodged cap that fully seals the airway is lethal within minutes; a cap with a hole through it still passes enough air to keep someone alive until help arrives. BIC and other manufacturers standardised the vent for exactly this reason, and international safety standards for writing instruments now specify minimum airflow through caps of a certain size.

Why it stays hidden

Because a safety feature that works leaves no evidence. The hole is invisible precisely in the cases where it did its job.
