A disease increases by 200 per cent (3 to 9 cases in 60 million). Why does the headline feel alarming?

A headline reports that a rare disease increased by two hundred per cent in one year. The actual numbers: from three cases to nine, in a country of sixty million. The percentage is accurate but creates a sense of epidemic, because two hundred per cent activates a magnitude frame that the base rate of three cases would not support.

What everyone sees

The percentage looks like a complete statistic. Two hundred per cent increase sounds catastrophic, and readers form their impression before encountering the base rate, if the base rate is provided at all. The format of the number does the framing: percentages emphasise change, absolute numbers emphasise scale, and the choice between them is rarely neutral.

What is actually happening

Gigerenzer and colleagues have argued that natural frequencies — absolute numbers — are processed more accurately than percentages, because the human cognitive system evolved to track counts rather than ratios. Slovic, Monahan and MacGregor showed that a statement like “20 out of 100 patients are violent” is perceived as more dangerous than “20 per cent of patients are violent,” because the frequency format evokes images of individuals. When a percentage change is presented without its base, the reader defaults to whatever base feels plausible, and plausibility is anchored by the magnitude of the percentage itself.

Why it stays hidden

The distortion hides because percentages feel self-contained. The format implies that all the information needed to evaluate the claim is inside the number: two hundred per cent is two hundred per cent regardless of context. This is mathematically true and psychologically false. Without the base, the reader constructs one, and the construction is influenced by the alarm the percentage has already produced.
